I left a medical doctor for this reason. I would be sitting in the exam room for 45 minutes or more as I hear him through the wall making phone call after phone call.

@chazmo has the right idea about making the first appointment of the day. That usually works like a charm.

The other factor is people with appointments before you all being late to their appointments pushing yours back.
