Welcome @melanie! I can't say exactly when your guitar was made, but it was some time after 1997, and when the Westerly RI plant closed in 2001. Guild's serial number records aren't complete for the late Westerly years. Hopefully @hansmoust will stop by and let us all know.

No... I bought it at the Guitar Center in Tampa - The sales guy was named Matt.
Correct - there was no case! But don't worry - I got a nice gator case for it the next day!
