The Guild Foxey Lady Fuzz by Mosrite and later Mike Matthews.
The padal was originally made by Mosrite based on the Mosrite Fuzzrite. Around 1967, Mosley came back from the NAMM show with an order from the Guild company to build Foxey Lady fuzztones for them.
Mike Matthews took over and built this pedal in 1968 for Guild, and also made the exact same pedal under the name "Axis".
Later models have a tone control added to them. Later on, Mike Matthews started the production of pedals under the name Electro Harmonix.

Wally did not get everything in his article. Not his fault, but he missed a few models along the way. Also, the original Model 60s all had two transformers, one for the power section and an additional transformer to provide 6.3 volts for the preamp filiments. He did the best he could with the limited information available.
